Vitality Weight Loss currently provides medical services only to patients located in the State of Nevada.
Acknowledgements you agreed to
- I am submitting this intake for asynchronous telehealth review by Dr. Peter DeBry, MD. I have read the risks and benefits of compounded semaglutide or tirzepatide and consent to treatment if and when Dr. DeBry approves my prescription based on my history.
- I understand my medication is compounded by a USA-licensed 503A pharmacy and is not FDA-approved, although the active ingredient is FDA-approved in brand-name form.
- I voluntarily assume the risks, release Vitality from liability except for gross negligence, and acknowledge receipt of the HIPAA Notice of Privacy Practices.
- I consent to clinical, appointment, and program text + email reminders. I can opt out of non-clinical SMS by replying STOP.
Informed consent summary
You're consenting to once-weekly subcutaneous injections of compounded semaglutide or tirzepatide, prescribed individually for you by Peter DeBry, MD (Nevada-licensed, NPI 1831137991) after asynchronous telehealth review of your intake.
What you can expect
Dr. DeBry reviews your intake within 24 hours. If approved, Dr. DeBry sends your prescription to a USA-licensed 503A compounding pharmacy, and we email you when it is ready to pick up. You self-administer weekly using the supplies provided with your prescription.
Common side effects
Nausea, vomiting, diarrhea, constipation, decreased appetite, fatigue, injection-site reactions. Usually most pronounced during dose increases.
Serious risks
Pancreatitis, gallbladder disease, kidney injury, hypoglycemia (especially with insulin/sulfonylureas), severe allergic reactions, diabetic retinopathy complications, mood changes. Boxed warning for thyroid C-cell tumors (do not use if MTC/MEN-2 history). Tirzepatide may reduce oral contraceptive effectiveness for 4 weeks after starting and after each dose increase.
Compounded medication disclosure
Your medication is prepared specifically for you by a USA-licensed 503A patient-specific compounding pharmacy based in Las Vegas, NV.
This pharmacy operates under Section 503A of the Federal Food, Drug, and Cosmetic Act. Each prescription is compounded individually under Dr. DeBry's individual order for you. Compounded medications are not FDA-approved as finished drug products, although the active ingredient is FDA-approved in brand-name form.
You may decline the compounded version and request the FDA-approved brand product through your own physician. Vitality is happy to coordinate.
Assumption of risk
I voluntarily assume the risks of this program, including GI side effects, pancreatitis, gallbladder disease, kidney issues, hypoglycemia, allergic reactions, retinopathy complications, mood changes, injection-site reactions, and risks specific to a compounded (non-FDA-approved) medication.
To the fullest extent permitted by Nevada law, I release Vitality Weight Loss, LLC and Peter DeBry, MD from claims arising from my voluntary participation, except those arising from gross negligence or willful misconduct. Governing law: Nevada. Disputes resolved in Clark County, Nevada.
HIPAA Notice of Privacy Practices summary
Vitality is a HIPAA covered entity. PHI is used for treatment, payment, and operations. Your rights include access, amendment, restriction, accounting of disclosures, and filing a complaint with Vitality's Privacy Officer or HHS Office for Civil Rights without retaliation.
Communications consent
Vitality may send appointment reminders, dose reminders, follow-up check-ins, educational content, and billing receipts via text and email. Message frequency varies; data rates may apply. Reply STOP to opt out of non-clinical SMS, or unsubscribe in emails. Clinical messages from Dr. DeBry may continue as needed for your safe care.
Telehealth (Nevada NRS 629.515)
Your care includes asynchronous (store-and-forward) telehealth with Dr. DeBry. This is not for emergencies: call 911 for emergencies, or 988 for mental-health crisis. Information shared via telehealth is treated as protected health information under HIPAA and Nevada law.
Billing & cancellation policy
Pricing depends on your medication: semaglutide is $300/month flat; tirzepatide is dose-tiered ($299/$349/$399/$449/$479/$499 by month of titration).
Billing is processed through Clover when you submit and at each monthly renewal. Cancel any time by calling 702-602-5002 or emailing care@vitalityacademies.health. Cancellation is effective at the end of the current billing period.
If Dr. DeBry declines you at clinical review: your payment is fully refunded. If you cancel after your prescription has been issued: the current month is non-refundable; future months are not billed.
Attestations you signed
- I certify that the health and personal information I have provided is true, accurate, and complete to the best of my knowledge. I understand that providing false information may result in denial of care and could endanger my health.
- I understand that Vitality Weight Loss and the prescribing physician may request a valid government-issued photo ID or other verification of my identity at any time, and may decline, delay, or discontinue treatment if my identity or eligibility cannot be reasonably confirmed.
- I understand that completing this intake does not guarantee a prescription, and that the physician may determine that asynchronous treatment is not appropriate for me.
